3. ELIGIBILITY REQUIREMENTS
Prospective bidders are required to submit the following documents:
(i) Certificate of Incorporation issued by the Corporate Affairs Commission (CAC), including Certified True Copy of Form CAC7 and CAC 2.1 or CAC1.1
(ii) Copy of Company’s Income Tax Clearance Certificate for the last three years (2017, 2018 and 2019) valid till 31st December, 2020;
(iii) Evidence of current Pension Compliance Certificate valid till 31st December, 2020;
(iv) Evidence of current Industrial Training Fund (ITF) Compliance Certificate valid till 31st December, 2020;
(v) Evidence of Registration on the National Database of Federal Contractors, Consultants and Service Providers by submission of valid Interim Registration Report (IRR) expiring 31/12/2020 or valid Certificate issued by BPP;
(vi) Evidence of current Nigeria Social Insurance Trust Fund (NSITF) Compliance Certificate valid till 31st December, 2020;
(vii) A Sworn Affidavit:
• disclosing whether or not any officer of the relevant committees of the Nigerian Maritime Administration and Safety Agency or the Bureau of Public Procurement is a former or present Director, Shareholder or has any pecuniary interest in the bidder and to confirm that all information presented in its bid are true and correct in all particulars;
• that none of the company’s directors has been convicted in any law court for any criminal offence including fraud and financial impropriety; and
• that company is not in receivership, bankrupt or insolvent.
(viii) Audited Accounts of the company for the last three years (2017, 2018 and 2019);
(ix) Evidence of financial capability to execute the project by submission of Reference Letter from a reputable commercial bank in Nigeria, indicating willingness to provide credit facility for the execution of the project when needed;
(x) Documentary evidence of at least three (3) similar jobs successfully executed/on-going within the last five (5) years including Letters of Award, Job completion certificate etc.);
(xi) Evidence of Re-capitalization (Applicable to Underwriters);
(xii) Company Profile with the signed Curriculum Vitae of Key Staff to be deployed for the project, including copies of their Academic/ Professional qualifications;
(xiii) Underwriters and Brokers Firm shall include Valid Operating License from National Insurance Commission (NAICOM);
(xiv) Underwriters and Brokers firms shall include NAICOM’s Approval for the Company’s account for year 2019/2020 (NAICOM);
(xv) Underwriters shall include valid Reinsurance /Treaty cover;
(xvi) Brokers shall include evidence of membership of Nigerian Corporation of Insurance Brokers;
(xvii) Brokers shall include valid Professional Indemnity;
(xviii) Premium payable shall be in Naira;
(xix) Period of Insurance/Cover is one (1) year or 365 days;
(xx) All documents for submission must be transmitted with a Covering/ Forwarding letter under the Company/Firm’s Letter Head Paper bearing amongst others, the Registration Number (RC) as issued by the Corporate Affairs Commission (CAC), Contact Address, Telephone Number (preferably GSM No.), and e-mail address. The Letterhead Paper must bear the Names and Nationalities of the Directors of the Company at the bottom of the page, duly signed by the authorised officer of the firm.

4. SUBMISSION OF EXPRESSION OF INTEREST
EOI documents shall be clearly marked “EXPRESSION OF INTEREST” stating the company name and the “Project Title”. The EOI shall be submitted in duplicate on or before 11:00a.m. on Thursday, 4 February, 2021 to the address below:

5. OPENING OF THE EOI
The EOI shall be opened immediately after the submission deadline at 11:00a.m. on Thursday, 4 February, 2021 at Multi-Purpose Hall of NMRDC, Kirikiri, Apapa. In line with social distancing directives by the Presidential Task Force on COVID-19, the presence of bidders shall not be required during the bid opening ceremony. The ceremony shall be covered by video recording, Relevant Professional Bodies and Non-¬Governmental Organizations (NGOs) are invited to witness the opening exercise and shall register evidencing their attendance on the date of the bid opening. 1. INTRODUCTION:
The SEPLAT, Operator of the NNPC/SEPLAT JV is seeking qualified Insurers for the Provision of Group Life Assurance Policy for OML 53. OML 53 covers an area of approximately 1,585km2 and is located onshore in the North Eastern Niger Delta. The Jisike oil field, located in the northwestern area of the block, is currently the only producing field on OML 53. SEPLAT invites submission from reputable Nigerian registered Insurance Companies having “world-class” experience in the provision of insurance services for major Oil and Gas companies.

2. SCOPE OF WORK:
The OML 53 Insurance cover is for the Group Life Assurance. The Insurer shall retain a percentage of the risk in Nigeria and re-insure the excess capacity with leading foreign underwriters with A+ rating by S & P. The Insurer will liaise with the jointly appointed re-insurance Broker for the purpose of re-insurance with foreign Underwriter. The insurer will also handle claims services, policy documentation, renewals, endorsement etc.

BASIC REQUIREMENTS SUPPORTING DOCUMENTS
Any Insurance Company seeking to be selected must satisfy the following conditions:(1)   Be a Limited Liability Company registered in Nigeria and licensed by the National Insurance Commission (NAICOM) to transact General and Life businesses.(2) Have a net asset of not less than N3billion as per its last published Accounts for its general business.(3) Have acquired good experience in Oil and Gas insurance business.(4) Have adequate and qualified staff with experience in Oil and Gas Insurance business.(5) Must be in good standing as regards prompt settlement of all claims reported to date.(6) Must be a good corporate citizen of Nigeria that pays its taxes as and when due. The interested Insurer must have the following documents :(1)   A certified true copy of the Corporate Affairs Commission (CAC) of the Company’s Certificate of Incorporation.(2)   A certified true copy by NAICOM of the Company’s license as an Insurer to transact Life, non-life and/or general Insurance business.(3)  Current registration with the Department of Petroleum Resources (DPR).(4)  Documentation/evidence demonstrating experience in similar Oil and Gas insurance cover that Insurer has provided in the last 3 years (2016 to 2018). 1.0 BACKGROUND
Kuara State signed into Law the Kwara State Mandatory Health Scheme Law in November 2017. This was done in order to establish In Kwara State a mandatory health scheme aimed at fostering improved access of residents to affordable and quality health care services.

This will in turn improve the health indices of Kwara State.

The scheme will offer a basic, defined minimum benefit package of health care services with optional top-ups, for individuals and families.

Kwara State Health Insurance Agency now seeks Expression of Interest from reputable, and experienced primary and secondary healthcare providers for the purpose of implementation of the Health Insurance Scheme

2.0 PROJECT SCOPE
The health care services provider will be expected to:
• Provide services as defined in the Kwara State Health Plan benefit package
• Comply with the Scheme’s Operational Guidelines and Standard Treatment Guidelines
• Provide returns on utilization of services and other data to the Kwara State Health Insurance Agency in compliance with stipulated guidelines.
• Institute an effective complaint management system and report any unresolved complaints to Kwara State Health Insurance Agency Adhere to approved Quality Improvement Plans and institutionalization of internal quality management systems
• Other responsibilities to ensure the viability of the program as may be determined by Kwara State Health Insurance Agency from time to time.

Search for Common Ground is an International, non-sectarian, nonprofit organization partnering with people around the world to ignite shared solutions to destructive conflicts. Search work cut across all levels of society to build sustainable peace through three main avenues: Dialogue+ Media+ Community.

Search has been working in Nigeria since 2004, using innovative approaches to promote peace building in areas of tension and to encourage understanding across ethnic, religious, and gender lines. With offices in Abuja, Jos, Maiduguri, Yola, Bayelsa and Warri, we are working to address conflict, utilizing consensus- based advocacy training, conflict resolution training, human rights monitoring and reporting and the facilitation of dialogue processes that engage all levels of society.

In order to be considered for pre-qualification, interested and competent contractors/suppliers shall submit the following:
(a) Company’s Profile.
(b) Evidence of registration with Corporate Affairs Commission.
(c) Evidence of Tax registration.
(d) Evidence of similar job carried out.
(e) Evidence of financial capability or bank support.
