INVITATION TO TENDER FOR 2022 CAPITAL PROJECTS
The Federal Teaching Hospital, Ido-Ekiti, a tertiary health institution, in implementation of her 2022 Appropriation Act, 2007 hereby invites all interested, qualified and reputable contractors with relevant experience and track record of excellence previous performance to tender for the following projects.
B. PROFILE OF PROJECTS
CATEGORY A: WORKS
|LOT R1||Construction of Gangway Connecting wards and Clinics|
|LOT R2||Construction of 150 Bedded Ward|
CATEGORY B: WORKS
|LOT R3||Procurement of computers systems and photocopiers|
|LOT R4||Procurement of furniture and fittings for office use|
|LOT R5||Upgrading of facilities in Hospital Annexes|
C. ELIGIBILITY REQUIREMENTS
a) Evidence of certificate of Incorporation issued by Corporate Affairs Commission (CAC) including Forms CAC1.1 or CAC2 and CAC7.
b) Evidence of firm’s Tax Clearance Certificates for the last three years valid till 31st December, 2022, with minimum average annual turnover of: works N100 million, goods N50million.
c) Evidence of Pension Clearance Certificate valid till 31st December, 2022 (this requirement is only applicable to bidders whose number of staff is 15 and above).
d) Evidence of Industrial Training Fund (ITF) Compliance Certificate valid till 31st December, 2022 (this requirement is only applicable to bidders whose number of staff is 5 and above or the bidder’s annual turnover is N50m and above);
e) Evidence of current Nigeria Social Insurance Trust Fund (NSITF) Compliance Certificate valid till 31st December, 2022;
f) Evidence of Registration on the National Database of Federal Contractors, Consultants and Service Providers by submission of Interim Registration Report (IRR) expiring on 31/12/2022 or valid Certificate issued by BPP;
g) Sworn affidavit disclosing
• That the required documents submitted are both genuine and up-to-date and that the hospital can verify the documents
• That none of the Directors of the company has ever been convicted by any court of law
• disclosing whether or not any officer of the relevant committees of the Federal Teaching Hospital, Ido-Ekiti or Bureau of Public Procurement (BPP) is a former or present Director, shareholder or has any pecuniary interest in the bidder and to confirm that all information presented in its bid are true and correct in all particulars;
• That the company is not bankrupt, or a party to liquidation proceedings in any court of law.
h) Company’s Audited Accounts for the last three years (2019, 2020, 2021), duly acknowledge by the Federal Inland Revenue (FIRS);
i) Verifiable documentary evidence of at least three (3) similar jobs executed in the last five (5) years including letters of awards, valuation certificates, job completion certificates and photographs of the projects;
j) Reference letter from a reputable commercial bank indicating willingness to provide credit facility for the execution of the project when needed;
k) Detailed Company’s profile with the curriculum Vitae and credentials of technical and management staff. Category A: Evidence of qualification of at least three (3) key technical personnel with relevant professional bodies such as COREN, QSDRBN, ARCON, CORBON etc;
l) Category b: Lot R3: certificate of registration from National information Technology Development Agency (NITDA)
m) Category b: Lot R5: letter of authorization from the original Equipment manufacturers listed in the approved policy for procurement of Health and Medical Equipment for Tertiary Hospitals in Nigeria
n) Category A: List of Plants/Equipment with proof of Ownership/Lease agreement;
o) For Joint Venture/Partnership, Memorandum of Understanding (MoU) should be provided (CAC, Tax Clearance Certificates, Pension Clearance Certificate, ITF Compliance Certificate, NSITF Clearance Certificate, IRR & Sworn affidavit are compulsory for each JV Partner);
p) All documents for submission must be submitted with a Covering/Forwarding letter under the Company’s letter head paper bearing amongst others, the Registration Number (RC) as issued by the CAC, Contact Address, Telephone Number (preferably GSM No) fax and email address. The letter head paper must bear the Names and Nationalities of the Directors of the company at the bottom of the page, duly signed by the authorized officer of the firm
D. COLLECTION OF TENDER DOCUMENTS
The tender documents are to be collected from procurement office (room 13 Administrative Building) FTH, Ido Ekiti between the working hours of 8:30am to 4:00om daily upon the payment of non-refundable tender fee of N10,000.00 per LOT into the Federal Teaching Hospital, Ido-Ekiti’s REMITA’s account and presentation of the copy of the Hospital’s receipt. The photocopy of the receipt should be attached to the tender documents.
E. SUBMISSION OF TENDER DOCUMENTS
Completed tender document should be two hard copies (Marked “Original & Copy) each for both Technical & Financial Bids with soft copy of financial bid only in MS Excel format, packaged in two separate sealed envelopes clearly marked at the top right corner of each envelope stating the title of the project i.e. TECHNICAL BIDS FOR LOT 1 & FINANCIAL BIDS FOR LOT 1, and at the reverse side the name of the company should boldly be written. Both envelopes shall then be put in a large envelope clearly marked at the top right corner stating the title of the project and the name and address fo the company at the reverse side and be left in the appropriate tender box at procurement office (Room 13, Administrative building) to be addressed to:

The submission should be made not later than 12:00 noon on Tuesday, 6th September, 2022. Late application will be returned unopened. Please make sure you signed Bid Submission Register at Procurement Office Room 13 Ground Floor Administrative Building FTH Ido-Ekiti, as Federal Teaching Hospital Ido-Ekiti will not be held responsible for misplaced or wrongly submitted bids.
F. OPENING OF TECHNICAL BIDS
Only the technical bids will be opened immediately after the deadline for submission at 12:00 noon on Tuesday, 6th September, 2022 at the Hospital’s MINI CONFERENCE HALL (old school of Nursing Hall).
i. Bids must be in English Language and signed by an official authorized by the bidder;
ii. All documents (Sensitive and non-sensitive) submitted would be verified. Bidders are advised to ensure integrity of all documents submitted
iii. The evaluation of the received documents shall be strictly based on the requirements specified in this Invitation to Tender.
iv. The submitted bids should be orderly and well paginated with table of contents particularly Technical bids, any bid that are not in order shall not be accepted
v. Bidders should not bid for more than two (2) lots
vi. Incomplete Bids shall not be accepted
vii. Any financial bids without softcopy(ies) Specified format shall not evaluated
viii. All documents submitted will be subjected to due diligence checks
ix. This publication is form information purpose only and does not constitute an offer by the Hospital to transact with any party for the project, nor does it constitute a commitment or an obligation on the part of the Hospital to award the contract to any to any contractor or agent.
x. The Hospital will not be responsible for any cost of expenses incurred by any interested company(ies) in connection with any response to this invitation and/or preparation or submission of the documents
xi. FTH, Ido-Ekiti also reserves the right to annul the procurement process at any time without incurring any liabilities in accordance with section 28 of the Public Procurement ACT 2007.
xii. Attendees at the bids opening shall be required to wash hands, use hand sanitizer, wear mask and observe physical distancing. The Bids Opening will recovered by video recording and invitation link will be sent to bidders who choose to join the bid opening online.
xiii. Bid document can also be collected in soft copies and Bidders can submit through Post (Courier Services)
xiv. Only pre-qualified bidders at technical evaluation will be invited at later date for virtual financial bids opening, while the financial bids of un-successful bidders will be returned un-opened;
