INVITATION TO TENDER
The Nnamdi Azikiwe University Teaching Hospital, Nnewi Anambra State, in furtherance of its statutory mandate intends to undertake the Procurement of Works and Goods under the 2020 Capital Appropriation and in compliance with the Public Procurement Act 2007 The Hospital hereby invites all experienced and competent Contractors, Suppliers and Consultants/Service Providers with relevant experience for consideration for the execution of the following projects.
2. SCOPE OF WORK
The scope of Works and Goods for the projects earmarked for execution is categorized as follows:
2.1. CATEGORY A: INVITATION TO TENDER FOR WORKS
LOT A1: Construction of NAUTH Permanent Site Internal Access Road (Phase 1)
LOT A2: Drilling of Motorized Borehole and construction of overhead tank at NAUTH Permanent Site
2.2 CATEGORY B: INVITATION TO TENDER FOR GOODS
LOT B: Provision/Installation of Office Furniture.
3. ELIGIBILITY REQUIREMENTS
Interested and competent Companies wishing to carry out any of the above projects are required to submit the following documents in their Technical Bids:
i. Evidence of certificate of incorporation with the Corporate Affairs Commission (CAC) including Form CAC1.1 or CAC2 and CAC7.
ii. Evidence of Company income Tax Clearance Certificate for the last three (3) years valid till 31st December, 2020
iii. Evidence of current Pension Compliance Certificate valid till 31st December, 2020 Evidence of current Industrial Training Fund (ITF) Compliance Certificate valid till 31st December, 2020.
iv. Evidence of current Nigeria Social Insurance Trust Fund (NSITF) compliance certificate valid till 31st December 2020
v. Evidence of Registration on the National Database of Federal Contractors Consultants and Service Providers by submission of Interim Registration Report (IRR) expiring on 30 June, 2020 or valid certificate issued by BPP
vi. Sworn Affidavit – Disclosing whether or not any officer of the relevant committees of the Nnamdi Azikiwe University Teaching Hospital or the Bureau of Public Procurement is a former or present Director, shareholder or has any pecuniary interest in the bidder and to confirm that all information presented in its bid are true and correct in all particulars
– The firm is not in receivership insolvent, android bankrupt
– The firm does not have any Partner who has ever been convicted in any court for a criminal offence in relation to Fraud or Financial Impropriety
vii. Company’s Audited Accounts for the last three (3) years 2017/2018/2019
viii. Evidence of financial capability to execute the project by submission of Reference letter from a reputable Commercial Bank in Nigeria indicating willingness to provide credit facility for the execution of the project when needed;
ix. Company’s Profile with the Curriculum Vitae of key staff to be deployed for the project including topics of their Academic/Professional qualifications
x. Verifiable documentary evidence of at least three (3) similar jobs executed in the last (5) years including letters of awards, Valuation Certificate, Job Completion Certificates and Photographs of the project.
xi. For Joint Venture/Partnership, Memorandum of Understanding (MoU) should be provided (CAC, Tax Clearance Certificate, pension Compliance Certificate, ITF Compliance Certificate, NSITF Compliance Certificate, IRR & Sworn Affidavit are compulsory for each JV Partner);
xii. All documents for submission must be transmitted with a covering forwarding letter under the company/firm’s letterhead paper bearing amongst others, the Registration Number (RC) as issued by the Corporate Affairs Commission (CAC), Contact Address, Telephone Number (preferably GSM No) and email address. The letter head Paper must bear the names and Nationalities of the Directors of the company at the bottom of the page, duly signed by the authorized officer of the firm.
4. COLLECTION OF TENDER DOCUMENTS FOR WORKS AND GOODS
Interested companies are to collect the Standard Bidding Document (SBD) at the office of the Head of Procurement Unit, NAUTH, Nnewi after the payment and presentation of the original copy of receipt of a non-refundable tender fee of ten thousand naira (N10,000.00) only per lot, made payable to Nnamdi Azikiwe University Teaching Hospital, Nnewi Treasury Single Account (TSA) through Remita, Account Number 0250189461018.
5. SUBMISSION OF BIDDING DOCUMENTS
Prospective bidders are to submit a bid for each of the desired Lot, one (1) original and two (2) hard copies each of the Technical and Financial bids with softcopy of financial bid only in MS Excel format, packaged separately in sealed envelopes and clearly marked as Technical Bid” or “Financial Bid “Both envelopes containing the Technical and Financial documents should be in one sealed envelope with the Project title, Company’s Name and Address Indicated at the back of the envelope and addressed to The Chief Medical Director, NAUTH Nnewi Anambra State and clearly marked with the name of the project and the Lot Number and drop in the designated Tender Box at the office of the Chief Medical Director, NAUTH Nnewi, Anambra State not later than 12:00 noon on Tuesday 23 June, 2020.
6. OPENING OF TECHNICAL BIDS
Only the Technical Bids shall be opened immediately after the deadline for submission at 12:00 noon on Tuesday 23rd June, 2020 at the Theatre complex of NAUTH, Nnewi, Anambra State in the presence of bidders or their representatives, while the Financial bids will be kept un-opened. Please, ensure that you sign the Bid submission register in the office of the Head of Procurement Unit, NAUTH Nnewi, as the Hospital will not be held liable for misplaced or wrongly submitted bids. 7. GENERAL INFORMATION
a. Bids must be in English Language and signed by an official authorized by the bidder.
b. Bids submitted after the deadline for submission would be returned un-opened.
c. Bidders should not bid for more than one (1) Lot
d. All cost will be borne by the bidders:
e. Original document or any aspect of the company’s submission may be requested for Sighting
f. Only the pre-qualified bidders will be contacted, at a later date for financial bids opening while financial bids of unsuccessful bidders will be resumed un-opened;
g. The Nnamdi Azikiwe University Teaching Hospital, Nnewi Anambra State is not bound to pre-qualify any bidder and reserves the right to annul the procurement process at any time without incurring any liabilities in accordance with section 28 of the Public Procurement Act 2007.
