INTRODUCTION
The University College Hospital, Ibadan is to provide Achievable, Realistic and Sustainable projects that will be beneficial to both patients and members of staff in providing excellent service delivery.
The University College Hospital, Ibadan hereby invites interested, competent and reputable contractors with relevant experience and good track records to submit bids for the under listed projects:

ELIGIBILITY REQUIREMENTS
Interested companies are requested to submit the following documents/information with verifiable evidences for assessment/consideration:
a) Evidence of Certificate of Incorporation issued by the Corporate Affairs Commission (CAC) including form CAC1.1 or CAC2 and CAC7;
b) Evidence of Company’s Income Tax Clearance Certificate for the last three (3) years 2018, 2019 and 2020 valid till 31st December, 2021;
c) Evidence of current Pension Clearance Certificate valid till 31st December, 2021;
d) Evidence of Current Industrial Training fund (ITF) Compliance Certificate valid till 31st December, 2021;
e) Evidence of Current Nigeria Social Insurance Trust Fund (NSITF) Clearance Certificate valid till 31st December, 2021;
f) Evidence of Registration on the National Database of Federal Contractors, Consultants and Service providers by submission of Interim registration Report (IRR) expiring on 31/12/2021 or valid Certificate issued by the Bureau of Public Procurement (BPP);
g) Sworn Affidavit disclosing whether or not any officer of the relevant Committee of the University College Hospital, Ibadan or Bureau of Public Procurement is a former or present Director, shareholder or has any pecuniary interest in the bidder and to confirm that all documents submitted are true and correct in particulars;
h) Company’s Profile with the Curriculum Vitae of Key Staff to be deployed for the Project including copies of their Academic/Professional qualifications such as COREN, ARCON, QSRBN, CORBON, etc.;
i) Company’s Audited Accounts for the last three (3) years i.e. 2018, 2019 & 2020;
j) Evidence of Financial Capability to execute the project by submission of Reference latter from reputable commercial bank in Nigeria, indicating willingness to provide credit facility for the execution of the project when needed;
k) Verifiable documentary evidence of at least three (3) similar jobs executed in the last five (5) years including Letters of Awards, Valuation/Job completion Certificates and Photographs of the Projects;
l) Lots 2 & 3: Letter of Authorization from the Original Equipment Manufacturers (OEMs) listed in the Approved Policy for Procurement of Health and Medical Equipment for tertiary hospitals in Nigeria;
m) Lot 4: Letter of Authorization as representative of the Original Equipment Manufacturers (OEMs)
n) For Joint Venture/Partnership, Memorandum of Understanding (MOU) should be provided (CAC, Tax Clearance Certificate, Pension Compliance Certificate, ITF Compliance Certificate, NSITF Compliance Certificate, IRR, Sworn Affidavit are compulsory for each JV Partner);
o) All documents for submission must be transmitted with a Covering/Forwarding Letter under the Company/Firm’s Letter Head paper bearing amongst others, the Registration Number (RC) as issued by the Corporate Affairs Commission (CAC), Contact Address, Telephone Number (Preferably GSM No.) and e-mail address. The Letterhead Paper must bear the Names and Nationalities of the Directors of the Company at the bottom of the page, duly signed by the authorized officer of the firm.

COLLECTION OF TENDER DOCUMENTS
Interested Companies are to collect the Standard Bidding Documents (SBD) from the office of the Assistant Director (Procurement) 2nd Floor Mortuary Building on evidence of payment of non-refundable tender fee of N10, 000.00 (Ten thousand naira only) into the UCH TSA REMITTA ACCOUNT in any reputable Commercial Bank in Nigeria. The teller for this payment should be converted to the Hospital’s receipt at the Finance & Accounts Department of the Hospital. Photocopy of the receipt should be attached to the Standard Bidding Documents (SBD).

ELIGIBILITY REQUIREMENTS
Interested and competent Contractors, Suppliers and Service providers are required to submit their application and technical bid containing the following:
i. Evidence of Certificate of Incorporation/Registration issued by Corporate Affairs Commission(CAC) including Forms CAC1.1 or CAC2 and 7;
ii. Evidence of Company’s income Tax clearance certificate for the last three (3) years [2018-2020), expiring on 31st December, 2021 corresponding with declared annual turnovers;
iii. Evidence of current Pension Clearance Certificate valid till 31st December, 2021;
iv. Evidence of current Industrial Training Fund (ITF) Compliance Certificate valid till 31st December, 2021;
v. Evidence of current Nigeria Social Insurance Trust Fund (NSITF) Clearance Certificate valid till 31st December, 2021;
vi. Evidence of Registration on the National Database of Federal Contractors, Consultants and Service Providers by submission of Interim Registration Report (IRR) expiring on 31/12/2021 or valid Certificate issued by BPP;
vii. Sworn Affidavit:
• disclosing whether or not any officer of the relevant Committees of Forestry Research Institute of Nigeria and its Colleges or Bureau of Public Procurement is a former or present Director, shareholder or has any pecuniary interest in the Bidder and to confirm that all information presented in its bid are true and correct in all particulars;
• that no Partner/Director have been convicted in any Country for criminal offences related to fraudulent or corrupt practices or criminal misrepresentation or falsification of facts relating to any matter,
• that the Company is not under receivership, the subject of any form of winding up petition or proceedings, insolvency or Bankruptcy
viii. Company Audited Accounts for the last three (3) years (2018, 2019 & 2020);
ix. Company’s Profile which must include company’s name and address, names of technical professionals and key personnel including copies of their qualification certificates and resumes. (in case of construction, COREN, QSRBN, ARCON, CORBON etc);
x. Verifiable documentary evidence of at least three (3) similar jobs completed within the last five (5) years with letter of Awards, Valuation/Practical Completion Certificates and photographs of the project;
xi. Evidence of financial capability to execute the project by submission of reference letter from a reputable commercial bank in Nigeria indicating willingness to provide credit facility for the execution of the project when needed.
xii. Works: List of Plants/Equipment with proof of Ownership/Lease.
xiii. FRIN 12: Letter of Authorization from the Original Equipment Manufacturers listed in the Approved Policy for Procurement of Health and Medical Equipment for Tertiary Hospitals in Nigeria:
xiv. Only companies included in the National Automotive Design and Development Council’s approved list of local manufacturers of vehicle or their authorized representatives should bid for vehicles
xv. All documents for submission must be transmitted with a Covering/Forwarding letter under the Company/Firm’s Letter Head Paper bearing amongst others, the Registration Number (RC) as issued by the Corporate Affairs Commission (CAC), Contact Address, Telephone Number (preferably GSM No.), and e-mail address. ELIGIBILITY REQUIREMENTS
Interested and competent Contractors wishing to carry out the above listed jobs are required to submit the following documents which will be subjected to verification by the Council:
i. Certificate of Incorporation issued by Corporate Affairs Commission (CAC). Please attach the company’s Article of Memorandum and duly certified forms CAC 1.1 or CAC 2 & 7. Business Name is also acceptable for Services;
ii. Evidence of Company’s Income Tax Clearance Certificate (or Personal Income Tax Clearance Certificates of all the Partners in case of Business Name) for the last three years (2018, 2019, 2020) expiring on 31st December, 2021, with minimum average annual turnover as stated in the Bid documents;
iii. Company’s Audited Accounts for the last three (3) years-2018, 2019 & 2020,
iv. Evidence of current Pension Clearance Certificate valid till 31st December, 2021;
v. Evidence of current industrial Training Fund (ITF) Compliance Certificate valid till 31st December, 2021;
vi. Evidence of current Nigeria Social Insurance Trust Fund (NSITF) Clearance Certificate valid till 31st December, 2021;
vii. Evidence of Registration on the National Database of Federal Contractors, Consultants and Service Providers by submission of Interim Registration Report (IRR) expiring on 31/12/2021 or valid Certificate issued by BPP:
viii. A Sworn affidavit:
• disclosing whether or not any officer of relevant committees of National Agricultural Seed Council or the Bureau of Public Procurement is a former or present Director, shareholder or has any pecuniary interest in the Bidder and to confirm that all information presented in its bid are true and correct in all particulars;
• that no Partner/Director have been convicted in any Country for criminal offences related to fraudulent or corrupt practices or criminal misrepresentation or falsification of facts relating to any matter;
• that the Company is not under receivership, the subject of any form of winding up petition or proceedings, insolvency or Bankruptcy:
xi. Verifiable documentary evidence of at least three (3) similar jobs executed in the last five (5) years including Letters of Awards, Valuation Certificates, Job Completion Certificates and Photographs of the projects;
xii. Evidence of financial capability to execute the project by submission of Reference Letter from a reputable commercial bank in Nigeria, indicating willingness to provide credit facility for the execution of the project when needed;
xiii. Company’s Profile with the Curriculum Vitae of Key Staff to be deployed for the project, including copies of their Academic/Professional qualifications such as COREN, QSRBN, ARCON, CORBON etc.);
xiv. Works: List of Plants/Equipment with proof of Ownership/Lease;
xv. Category C: Evidence of Firm’s current registration with relevant regulatory professional body(ies);
xvi. Category C: Evidence of accreditation by Centre for Management Development (CMD);
xvii. Only companies included in the National Automotive Design and Development Council’s approved list of local manufacturers of vehicles or their authorized representatives should bid for the vehicles;
xviii. For Supply of Equipment: Letter of Authorization as representative of the Original Equipment Manufacturers (OEMs);
xix. All documents for submission must be transmitted with a Covering/Forwarding letter under the Company/Firm’s Letter Head Paper bearing amongst others, the Registration Number (RC) as issued by the Corporate Affairs Commission (CAC), Contact Address, Telephone Number (preferably GSM No.), and e-mail address. The Letterhead Paper must bear the Names and Nationalities of the Directors of the Company at the bottom of the page, duly signed by the authorised officer of the firm.

COLLECTION OF TENDER DOCUMENT (CATEGORY A & B)
Tender documents are obtainable from Procurement office, NASC Headquarters Sheda-Gwagwalada on the presentation of original receipt of non-refundable tender fee of N10,000:00 (Ten Thousand Naira) for each Lot paid into National Agricultural Seeds Council TSA/REMITA A/C.

SUBMISSION OF TENDER DOCUMENTS (CATEGORY A & B)
The completed tender documents must be duly registered at the Procurement Unit and submitted in three (3) copies, one (1) original and two (2) copies in two separate envelopes clearly marked “Technical” and “Financial”. Both Envelopes must be submitted in one large sealed envelope, mark with Lot number at the top left Hand corner and addressed to the Secretary Tenders Board, National Agricultural Seed Council, Sheda, Gwagwalada KM29 Abuja-Lokoja High way. Furthermore, the reverse of each sealed envelope should bear the name and address of the bidder and drop in the designated Tender Box not later than 12:00 noon on Monday, 7th June, 2021.

SUBMISSION OF EXPRESSION OF INTEREST (CATEGORY C)
Interested firms are to submit two (2) bound of Expression of Interest (EoI) documents separated by dividers and arranged as indicated above. The documents should be submitted in a sealed envelope and addressed as indicated above and clearly marked with (the name of the project and the Lot number). 1. INTRODUCTION
The Federal Government of Nigeria has allocated funds to the Federal Science and Technical College Ahoada, Rivers State, in the 2021 Appropriation Act for the Procurement of Works towards achieving the mandate of the College.
In compliance with the provisions of the Public Procurement Act, 2007, the College invites interested and eligible Contractors with relevant proven competence and experience to participate in the bidding process of the following Projects:

(e) Evidence of current Nigeria Social Insurance Trust Fund (NSITF) Clearance Certificate valid till 31 December, 2021;

(g) Sworn Affidavit:
– that the company is not in receivership, insolvent or bankrupt; that the company does not have any Director who has ever been convicted in any court in Nigeria or any other country for criminal offense in relation to fraud or financial impropriety;
– disclosing whether or not any officer of the relevant committees of Federal Science and Technical College, Ahoada or the Bureau of Public Procurement is a former or present Director, shareholder or has any pecuniary interest in the bidder and to confirm that all information presented in its bid are true and correct in all particulars;
(h) Audited Financial Statement of the company for the past three years (2018, 2019 and 2020) duly signed and sealed by relevant firm and the turnover as reflected in financial statements must correspond with those on the Tax Clearance certificate;
(I) Evidence of financial capability to execute the project by submission of Reference Letter from a reputable commercial bank in Nigeria, indicating willingness to provide credit facility for the execution of the project if awarded;
(j) Company’s Profile with the Curriculum vitae of Key Staff to be deployed for the project, including copies of their Academic/Professional qualifications such as COREN, QSRBN, ARCON, CORBON etc,
(k) Verifiable documentary evidence of at least three (3) similar jobs executed in the last five (5) years including letters of Awards, Valuation Certificates, Job Completion Certificates and photographs of the projects;
(l) List of Equipment with proof of Ownership/Lease;
(m) For Joint Venture/Partnership, Memorandum of Understanding (MoU) should be provided (CAC, Tax Clearance Certificate, Pension Compliance Certificate, ITF Compliance Certificate, NSITF Compliance Certificate, IRR & Sworn Affidavit are compulsory for each JV partner): and
(n) All documents for submission must be transmitted with a covering/forwarding letter under the Company/Firm’s Letter Head Paper bearing amongst others, the Registration Number (RC) as issued by the Corporate Affairs Commission (CAC), Contact Address and Telephone Number (preferably GSM No., and e-mail address. The Letterhead Paper must bear the Names and Nationalities of the Directors of the Company at the bottom of the page, duly signed by the authorized officer of the firm.

4. COLLECTION OF TENDER DOCUMENTS
The tender documents are to be collected from the Office of the Vice Principal Special Duties, FSTC, Ahoada between the working hours of 08:00am and 04:00pm daily upon the payment of non-refundable tender fee of N10,000.00 per Lot to Federal Science Technical College, Ahoada, Rivers State through REMITA platform in any Commercial Bank, and present the teller for the College receipt.

5. SUBMISSION OF TENDER DOCUMENTS
Prospective bidders are to submit bid for each of the lot desired, two Hard copies (Marked “Original & Copy”) each for both Technical & Financial Bids with soft copy of Financial bid only in MS excel format, packaged in two separated wax sealed envelopes clearly marked at the top right corner of each envelope stating the title of the project i.e. “TECHNICAL BIDS FOR project Title and LOT No.” & “FINANCIAL BIDS FOR Project Title and LOT No.” and at the reverse side the name of the company should be boldly written. Both envelopes shall then be put in a larger envelope clearly marked at the top right corner stating the title of the project and be left in the appropriate Tender Box at the Vice Principal Special Duties office to be addressed to:

a. Evidence of Certificate of Incorporation issued by Corporate Affairs Commission (CAC) including Form CAC 1.1 or CAC2 and CAC7. Business Name is acceptable for Services;
b. Evidence of Company’s Income Tax Clearance Certificate (or Personal Income Tax Clearance Certificates for all the Partners in case of Business Name) for the last three (3) years valid till 31st December2021, with a minimum average annual turnover as stated in Category B
